Assumption of Risk & Waiver of Liability

By registering for the Frontrunner Fieldhouse Futsal League, the participant (and parent/guardian if participant is a minor) acknowledges and agrees to the following:

ASSUMPTION OF RISK: I understand that participation in futsal and related activities involves inherent risks, including but not limited to: physical contact with other participants, falls, collisions, sprains, strains, fractures, concussions, and other injuries that may result from the nature of the sport. I voluntarily assume all such risks, both known and unknown, even if arising from the negligence of Frontrunner Fieldhouse, its owners, employees, coaches, referees, or other participants.

Release of Liability

RELEASE AND WAIVER: In consideration of being permitted to participate in the Frontrunner Fieldhouse Futsal League, I hereby release, waive, discharge, and covenant not to sue Frontrunner Fieldhouse, LLC, its owners, officers, directors, employees, agents, coaches, referees, volunteers, and affiliated organizations (collectively, "Released Parties") from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while participating in such activity, or while on the premises owned or operated by Frontrunner Fieldhouse.

Medical Authorization

MEDICAL TREATMENT AUTHORIZATION: I authorize Frontrunner Fieldhouse and its representatives to seek and consent to emergency medical treatment for the participant in the event of injury or illness during league activities, if a parent or guardian cannot be reached. I agree to be responsible for any medical expenses incurred.

Photo & Video Release

MEDIA RELEASE: I grant Frontrunner Fieldhouse permission to use photographs, video recordings, and other media of the participant taken during league activities for promotional, educational, and marketing purposes, including but not limited to social media, website content, and printed materials, without compensation or further consent.

Code of Conduct

All participants, parents, and spectators agree to:

• Treat all players, coaches, referees, and staff with respect
• Refrain from abusive language, threats, or unsportsmanlike conduct
• Support fair play and positive sportsmanship at all times
• Follow all facility rules and referee decisions
• Understand that violation of this code may result in removal from the league without refund

Refund Policy

REFUNDS: Registration fees are non-refundable once the season has begun. Requests for refunds prior to the season start date will be considered on a case-by-case basis and may be subject to an administrative fee. No refunds will be issued for missed games due to player absence, injury, or removal for code of conduct violations.

Acknowledgment

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, INCLUDING MY RIGHT TO SUE. I ACKNOWLEDGE THAT I AM SIGNING THIS AGREEMENT FREELY AND VOLUNTARILY, AND INTEND MY SIGNATURE (OR ELECTRONIC ACCEPTANCE) TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.

By completing registration, you acknowledge that you have read, understand, and agree to all terms outlined above on behalf of yourself and/or your minor child.
