Assumption of Risk Futsal is a physical sport involving running, jumping, and contact. Participants acknowledge the inherent risks of injury, including but not limited to sprains, fractures, concussions, and other bodily harm. By registering, you voluntarily assume all risks associated with participation. Waiver of Liability To the fullest extent permitted by law, you release and hold harmless Frontrunner Fieldhouse LLC, its owners, staff, coaches, referees, and volunteers from any and all claims, damages, or liability arising from participation in league activities, including injuries, property damage, or loss. Medical Authorization In the event of an emergency, you authorize Frontrunner Fieldhouse staff to seek emergency medical treatment on your behalf or on behalf of your team members. Minor Players (Under 18) Any player under the age of 18 participating in adult leagues MUST present a signed Minor Consent Form at the first game of the season. Minor Consent Forms are available at the front desk or by emailing leagues@frontrunnerfieldhouse.com. Players under 18 will NOT be permitted to play without a signed consent form on file. Code of Conduct All players, coaches, and spectators are expected to demonstrate respect for opponents, referees, staff, and fellow spectators. Unsportsmanlike behavior, verbal abuse, or physical altercations will result in immediate ejection and possible suspension from the league without refund. Referee decisions are final. Referee Fees Each team is responsible for paying $15 per game directly to the referee before kickoff. This is a team captain responsibility. Failure to pay referee fees may result in game forfeiture. Refunds Captain deposits and player registration fees are non-refundable once the season has begun. Requests for refunds prior to the season start date will be considered on a case-by-case basis and may be subject to an administrative fee. No refunds will be issued for missed games due to player absence, injury, or removal for code of conduct violations. Photo/Video Release Frontrunner Fieldhouse may photograph or video record league games and events for promotional, marketing, and social media purposes. By registering, you consent to the use of your likeness in such materials.
