WAIVER AND RELEASE OF LIABILITY
I, the undersigned parent or legal guardian of the above-named child, hereby grant permission for my child to participate in the Frontrunner Fieldhouse Move & Learn Summer Camp program. I understand that participation involves physical activity and inherent risks. I release Frontrunner Fieldhouse, its owners, employees, and volunteers from any and all claims, damages, or liabilities arising from my child's participation. I confirm that my child is physically able to participate and that all medical information provided is accurate. I authorize Frontrunner Fieldhouse staff to seek emergency medical treatment if needed. I consent to the use of photographs and videos of my child for promotional purposes.
I understand that participation in athletic and recreational activities involves inherent risks, including, but not limited to, slips and falls, collisions, muscle strains, sprains, broken bones, concussions, and other serious injury. I voluntarily choose for my child to participate and knowingly assume all such risks.
I agree to indemnify and hold harmless Frontrunner Fieldhouse LLC from any claims, demands, damages, costs, and attorneys' fees arising out of my child's involvement in the program.
In the event of illness, injury, or medical emergency, and if I cannot be reached, I authorize Frontrunner Fieldhouse staff to seek emergency medical treatment. I am responsible for all related costs.
I grant Frontrunner Fieldhouse permission to capture my child's image and likeness in photographs and video for promotional purposes without compensation. I may revoke this permission in writing.
Fees are non-refundable once the camp week begins, except at Frontrunner Fieldhouse's sole discretion.
This waiver remains in effect for the duration of the summer camp program 2026.
