WAIVER AND RELEASE OF LIABILITY

I, the undersigned parent or legal guardian, hereby grant permission for my child to participate in the Frontrunner Fieldhouse Little Kickers Pre-Academy program. I understand that participation involves physical activity appropriate for toddlers and preschool-age children and carries inherent risks. I release Frontrunner Fieldhouse, its owners, employees, and volunteers from any and all claims, damages, or liabilities arising from my child's participation.

I confirm that my child is physically able to participate and that all medical information provided is accurate. I authorize Frontrunner Fieldhouse staff to seek emergency medical treatment if needed. I consent to the use of photographs and videos of my child for promotional purposes. Monthly fees are due at the start of each month.
