FRONTRUNNER FIELDHOUSE - LIABILITY WAIVER, RELEASE, AND INDEMNITY AGREEMENT
By signing this agreement, I acknowledge and agree to the following terms and conditions on behalf of myself and my child(ren):
1. ASSUMPTION OF RISK
I understand that participation in athletic and recreational activities at Frontrunner Fieldhouse, including but not limited to soccer, futsal, general sports, arts and crafts, and related camp activities, involves inherent risks of physical injury. These risks include but are not limited to sprains, fractures, concussions, heat-related illness, and other injuries that may occur during physical activity. I voluntarily assume all risks associated with my child's participation.
2. RELEASE AND WAIVER OF LIABILITY
I, on behalf of myself, my child, and our heirs, executors, and assigns, hereby release, waive, and discharge Frontrunner Fieldhouse LLC, its owners, officers, employees, coaches, volunteers, agents, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or property damage sustained during participation in any program, camp, or activity.
3. MEDICAL AUTHORIZATION
In the event of an emergency, I authorize Frontrunner Fieldhouse staff to seek and obtain emergency medical treatment for my child. I understand that I am responsible for all medical expenses incurred. I have disclosed all relevant medical conditions, allergies, and medications on this registration form.
4. PHOTO AND VIDEO CONSENT
I grant permission to Frontrunner Fieldhouse to photograph and/or video record my child during programs and activities. These images may be used for promotional purposes, including but not limited to social media, website content, marketing materials, and press releases. No compensation will be provided for such use.
5. CODE OF CONDUCT
I understand that my child is expected to follow all rules and instructions given by Frontrunner Fieldhouse staff. Disruptive, violent, or unsafe behavior may result in dismissal from the program without refund.
6. DROP-OFF AND PICK-UP
I understand that I am responsible for timely drop-off and pick-up of my child. Frontrunner Fieldhouse is not responsible for supervision outside of designated program hours. Late pick-up may incur additional fees.
7. INDEMNIFICATION
I agree to indemnify, defend, and hold harmless Frontrunner Fieldhouse and its affiliates from any claims, damages, or expenses (including attorney fees) arising from my child's participation or my breach of this agreement.
8. GOVERNING LAW
This agreement shall be governed by the laws of the State of Texas. Any disputes shall be resolved in Tarrant County, Texas.
By typing my name below and checking the agreement box, I acknowledge that I have read, understood, and agree to all terms above. This electronic signature carries the same legal weight as a handwritten signature.
