FRONTRUNNER FUTSAL LEAGUE – Team Registration Waiver, Release of Liability & Consent

By submitting this registration on behalf of my team, I (the Team Manager/Captain) acknowledge and agree to the following terms:

Team Responsibility

As the registered Team Manager/Captain, I accept responsibility for ensuring all players on my roster understand the risks of participation and have agreed to participate voluntarily. All players must be 16 years of age or older. For any players ages 16-17 on the roster, the Team Manager certifies that written parental/guardian consent has been obtained prior to their participation.

Assumption of Risk

I understand that participation in futsal involves inherent risks including slips, falls, collisions, strains, sprains, and other injuries. I acknowledge these risks on behalf of myself and communicate them to my team, and we voluntarily choose to participate.

Release of Liability

I release Frontrunner Fieldhouse LLC, its owners, officers, employees, coaches, volunteers, and agents from any claims arising from participation in the League, including personal injury or property damage to any team member.

Medical Responsibility

Each player is responsible for their own medical decisions and costs. In case of emergency, players authorize Frontrunner staff to seek medical treatment as necessary.

Code of Conduct

I agree that all players and spectators associated with my team will show respect, follow rules, and refrain from inappropriate behavior. Frontrunner may remove anyone whose conduct is unsafe, disruptive, or unsportsmanlike.

Media Release

I grant permission for Frontrunner to capture and use images of team members for promotional purposes without compensation.

Refund Policy

Once the league begins, team registrations are generally non-refundable except at Frontrunner's discretion.

Manager/Captain Authorization

I certify that I am authorized to register this team and that all players are 16 years of age or older. For any minor players (ages 16-17), I confirm that parental/guardian consent has been obtained.

I certify that I am 16 years of age or older. If I am under 18 years of age, I confirm that my parent or legal guardian has provided consent for my participation in this adult league (see Minor Consent section above).
