Assumption of Risk

Futsal is a physical sport that involves running, jumping, kicking, and physical contact. By registering, I acknowledge that participation carries inherent risks including but not limited to sprains, fractures, concussions, and other injuries. I voluntarily assume all such risks on behalf of myself and/or my minor child.

Waiver of Liability

I hereby release and hold harmless Frontrunner Fieldhouse LLC, its owners, officers, employees, staff, coaches, referees, volunteers, and agents from any and all claims, damages, losses, or liabilities arising from participation in futsal leagues, games, practices, or related activities at the facility.

Medical Authorization

In the event of an emergency, I authorize Frontrunner Fieldhouse staff to seek and obtain emergency medical treatment for myself and/or my minor child. I understand that I am responsible for all medical expenses incurred.

Code of Conduct

All players, parents, and spectators are expected to demonstrate respect for teammates, opponents, coaches, referees, and facility staff at all times. Unsportsmanlike behavior, abusive language, or threatening conduct may result in immediate removal from the facility and forfeiture of registration fees without refund.

Refund Policy

Registration fees are non-refundable once the season has begun. Pre-season refund requests will be considered on a case-by-case basis and may be subject to an administrative fee. No refunds will be issued for missed games, regardless of the reason.

Photo / Video Release

I grant Frontrunner Fieldhouse LLC permission to photograph and/or video record participants during league play and related activities. These images may be used for marketing, social media, website content, and promotional materials without compensation.
